Specialist Support Coordination (Level 3)

Specialist Support Coordination is designed for participants with more complex needs or situations that require a higher level of expertise.

Our Specialist Support Coordinators can help when:

  • There are multiple services or providers involved

  • There are barriers to accessing supports

  • You require coordination across health, housing, or justice systems

  • You need tailored strategies for stability and progress

We work closely with you, your support network, and other professionals to ensure a coordinated, effective approach that meets your individual circumstances.
